Before you can book your session/s, please review our Terms and Conditions, look at our GDPR (General Data Protection Regulation) Document and ICRW (Informed Consent Release Waiver) Form (kids versions on the Fit Kidz Forms page and adult versions on the Fit Adultz & Fit Famz Forms page), and the Par-Qs (Physical Activity Readiness Questionnaire) for both kids and adults (links below to electronic forms).

The Par-Q is the only one we will need you to fully complete and submit but ONLY IF you need to answer ‘yes’ to any of the medical questions contained within the form. If not, we will assume you have no important medical information to share with Fit Kidz prior to commencing any physical activity with us, and that you take full responsibility should you encounter any medical issues in relation to anything you haven’t disclosed to us. Please note, if you are a returning customer and you’ve previously completed a Par-Q you do not need to do it again.
